If you would like a physical copy of your 1095-C, please inform Human Resources via email is people@10xhealthsystem.com, 631-944-2006, 2920 NE 207th St Ste 901 Miami FL 33180. 85254.

Please include your full name, contact information, and the year of the 1095-C that you are requesting. A paper copy will be mailed to the address on file, within 30 days of receiving your request. Note: The option in UKG to receive a paper copy is not considered a request for a copy of this form, and you must notify HR separately.
